<br />Making an appointment is easy, simply fill out the registration form below and one of our friendly staff will give you a call next working day. Consultation take up to 30 minutes. All applicants under the age of 18 will need to have this application submitted by parent or guardian.<br />

ACC Information

<br />ACC Provider Code: <strong>ORTHOPED/AK </strong>ACC applicants, please quote Orthopedica’s provider code <strong>ORTHOPED/AK </strong>on the referral letter from your Orthopedic Specialist or Surgeon prior to booking your appointment.<br />
